New Patient Appointments
Dr. Saadia sees new patients by physician referral only. This means all new patients are required to have a referral from their primary care provider or treating specialist. We require pertinent medical records to be submitted along with the referral. Once we are in receipt of the referral and medical records, the patient will be contacted to schedule an appointment.
In addition, please be advised Dr. Saadia is seeing new patients on a limited basis as the practice is reaching capacity. Unfortunately, this means Dr. Saadia will not be able to see everyone who is referred to her. She will review all incoming referrals. Our office will contact the referring provider if another referral should be made. We apologize for any inconvenience and appreciate your trust in Dr. Saadia.

If we asked you to complete a medical record request, please print the form and mail, fax or deliver it to our office as soon as possible. We need to obtain these records prior to your appointment.
